Guides and channel numbers

• Numbers start at 101, so they never clash with antenna channels (2.1 to 69.x).
• Channels are numbered in blocks of 100 by category, and a channel keeps its number.
• Guides come from each broadcaster's own published schedule. A few broadcasters don't publish one, so their channels show without programme names.
